There's a bug with the buff not being properly applied. I notice it happening often when dying/ressing, but am not quite sure what triggers it. It might be they had the buff during the kill, and lost it afterwards, or just some weird issues. I have lost quite a lot of shards by just losing the buff early in an instance.
The patchnotes for 3.0.8 state that it's being fixed.
