I've never been too picky with what I eat, but I used to have a certain "blech" attitude to some stuff.
That has changed since becoming a chef. As a Chef, you need an open mind if you want to be able to make good delicious meals, and that just wont happen if you refuse to eat what you make. Because of that, previous edible sources of food that I would had considered disgusting in the past, I eat with gusto today.
There is one exception, but its more for health issues than anything. I can't eat zucchini. While I'm not particularly a fan of how it tastes, I don't dislike it either, but even a small bite, produces me A LOT of gases, and make me spend the rest of the day, and a good chunk of the next 2 days, constantly burping (or you know, releasing them the "other" way). So because of that, I can't eat any zucchini.
Insects are an interesting topic, I used to feel gross about them (like 99% of the planet) but I went to a conference about native-venezuelan cuisine, with a Yanomami Chef (Yanomami is one of the native aborigenous tribes that still live in the deep jungles of venezuela), where the main courses were Tarantula Croquettes, Worm Meatloaf, and Roasted Crickets in Honeyed Sauce.
They tasted like GLORY.
I was VERY VERY VERY hesitant to even smell that stuff, let alone taste it. But with the encouragement of my Instructor (Who's become an inspiring mentor figure for me) I tried it, and loved it. Haven't made any of those recipes, mainly because the ingredients are hard to procure (come on, where can you get Tarantula Legs? or Bachezetto Worms?). But I would if I could.
Non-fish seafood (clams, shrimp, squid, etc.). I love all kinds of fish though.
Oh yeah, even if shrimps for example tasted good, they look like giant insects. Ew.
What if you eat them when they're broiled, finely chopped, and mashed into puree? Then sautéed in a veggie mixup with soy sauce and a pinch of black pepper? They don't look like insects anymore.

I don't like the taste of raw onions. If they have been grilled, sauteed or steamed and put on something, I like them. Like a cheesesteak or patty melt (I plan on making some patty melts this upcoming week for dinner. They were a lot of work to make, but they were delicious.)

Mainly pork, for religious reasons. I'm not a big fan of okra, either. Like, at all.

Olives and brown beans. Which is kinda weird, because i prepare a lot of food with olive oil.
I'm not a big fan of most fish either. Though it really depends on how it's prepared.
I'm not that picky really.
A friend of mine though has what can only be described as a mortal fear of cheese (he's not allergic). Any cheese. French, Dutch, yellow, green, blue. Up to the point where he refuses to eat something prepared on a surface that previously helt cheese.
But at some point we were on holiday in Bulgaria and he ordered a dish that contained cheese. I don't know if he glanced over it or missed the translation something, but me and some other guys had seen it on the menu. We decided not to say anything and have some fun.
So the guy starts eating and after half a plate we asked if it was any good. Yes, was the answer, and we start giggling about this. So he asks us what's up and we read him the contents of his meal. He pushes his plate away, runs to the bathroom to stick his finger down his throat....

fresh tomatoes and I don't mind pureed tomatoes (non-chunky pasta and pizza sauce, for example) at all, but I literally gag on chunks of cooked tomatoes in my sauce or other food.

Gonna agree here. While I prefer cooked vegetables (dangit, I consider tomatoes as vegetables), I highly dislike cooked tomatoes.

For me the three things I avoid are bananas, eggs mushrooms. I've never liked them and decided recently to try both again because tastebuds change. Nope. Nuh uh. Mushrooms are gross, and with bananas and eggs even the smells make me gag. They are both okay IN things if the taste isn't too much, but alone or as a main ingredient, nope. Everyone laughs that I have chickens and don't like eggs.
I also avoid several foods - mainly peanuts and tree nuts - when I'm with my boyfriend. He has severe food allergies to a multitude of foods but peanuts, tree nuts and eggs (which I don't eat anyways) are among the worst so I don't eat them at all if we are together. The other foods he is allergic to I will eat but wash my hands (and sometimes, brush my teeth) immediately after eating them to avoid cross contamination.

Ugh, that's the worst. I'm allergic to tree nuts as well, but most things made with peanuts are made on equipment that does tree nuts, so I can't have them either >_<

Yeah, my boyfriend can't have 99% of processed foods because it either comes into contact with the things he is allergic to, or contains it. In addition to the peanuts/treenuts and eggs, he can't have soy, sunflower/safflower, beans and other legumes and dairy. You don't realize just how many things contain sunflower/safflower oil or soy until you deal with deadly allergies to them.
Food allergies are something I never paid attention to before... I'm glad I'm educated now, because it's quite a serious issue that many people don't take seriously unfortunately :(

Yeah a lot of people mentioned eggs, and I was the same for a few years. I got back into them eventually, although hard boiled eggs aren't my favorite... I used to drink raw eggs as well. People used to keep telling me it was disgusting, lol. Mushrooms is another one quite a few of you mentioned. I LOVE mushrooms.

That's because it is. Blech. Not to mention a rather quick way to catch salmonella.
I like eggs, but always cooked in one way or another. I'm not particularly fond of dishes who's main ingredient is egg, but when egg is a part of a whole, it can add body, and flavor to a preparation. I know a delicious french recipe, called "Terrina a las tres carnes" that involved mixing 3 types of meat (Pork, Chicken, and Beef) with eggs and make a meatloaf out of it with bacon.
I swear to God almighty its one of the most delicious recipes I've made.
