Me, I use this:
IT->PS->Pestilence->DnD->UB
Unfortunately, in order to have enough power for UB I need to have IT glyphed though.

EDIT:
Once my runes come back, I spam death strike with my frost/unholy runes, blood boil with my blood runes, and death coil with my runic power.

if you're DPS, you can use plague strike (to proc desecration) -> icy touch -> pestilence -> death and decay.

As a tank, I always drop death and decay first, as the icy/plague/pestilence take much longer to build threat on the targets.
