Generally, if you're fighting in an encounter where there's an "enrage" or sudden increase of damage to you you'll want to save them for those times (such as Menexxa web stun etc.).
Should you be cruising on easy ground however (as if doing content you outgear) then you can either pop at consequtively at all time (giving you a very smooth damage taken ratio).
Or, should you be having trouble with threat (and you're kinda "safe"), ignore them until things are looking grim.

as far as DK CD's use UA when you have a lot of adds, as it takes damage off per hit, and is particularly useful when there are 10adds pounding on you.
Use IBF if you are expecting a big hit.
and use AMS if you are expecting a big magic hit
Or use them if your aren't expecting adds/a big hit to come up during thier CD to reduce general damage taken and healer strain.

the different DK cooldowns can be saved for when you're expecting larget hits.
Basically what I do when raiding is trying to keep a CD up most of the time, that beeing IBF, UA and my Dodge Trinket, simply because otherwise you'll often get to that point where you'll use them so rarely, that it's not good either.
Of course on some bosses they are better kept, because you know when more dmg is done (ie Emalon add overcharge).
You'll probably find out what suites you the best, but that is just what i think.
