Honestly, for me it's my Swift Flight Form The amount of work it took me back in Burning Crusade to made the 5000g to start the quest, then Heroic Sethekk Halls, I felt so accomplished when I got it.
My favorite and the one I have the most proud of having is Stood in the fire. I had just started to play WoW, and I was questing at Stonetalon Mountains, when everything turned RED, and I was like "Oh, my god!", and just ran into Deathwing's flames with a smile on my face :)

Without a doubt my most favorite achievement has to be Three's Company: 2700 because it reminds me how many nerds I've slain to obtain it.
It's a tie between What a Long, Strange Trip It's been and Bane of the Fallen King. I don't PvP so I had to really work on dragging my orphan through the Battleground. Took me 3 yrs to wrap that up. Bane of the Fallen King (Before Cata Launch), because it showed me how great a good raid team can be. :)
For me... it was standing in line for 2 hours to make sure I got Collector's Edition: Mini-Diablo i've collected all of the collectors editions since.
I'm most proud of "The Loremaster" which I completed in Summer 2010! :) I've had that toon since June 2007 so completing all those quests over the years and finally crunching last summer to get the title and tabard was such a fun experience, and definitely one of my most proud achievements! <3
My proudest achievement, by far, is Veteran of the Shifting Sands. I wasn't yet playing when this represented relevant content, but my own interest in the lore of the game drove me to complete the epic, and sometimes irritating, quest chain anyway. Particularly irritating was grinding out another TEN Elementium Ingot, back before they'd become "ingots." You see, I'm still working on my Thunderfury, and, before taking on the quest chain that lead to this achievement, I had already obtained the necessary arcanite and other materials (including the skill) needed to smelt my first 10 Elementium Ingot into Enchanted Elementium Bar. I discovered, on Wowhead, if memory serves, that there is a certain pack of mobs that can be skipped and farmed in BWL, as long as the person doing the farming reenters the raid within the first half hour (preventing the soft reset from occurring). After clearing the raid with a pug whose primary concern was killing Nefarian, I spent the next several hours farming this one pack of mobs until I had obtained the necessary "ingots." As the Shattering approached, I found myself forced to solo the Moonglade event. As an 80 prot pally, surviving wasn't the issue, but keeping Keeper Remulos alive proved a hassle. The following week, I was set into a panic because, thinking I'd left Eranikus and his assault on Moonglade for last, I found myself unable to advance to the final (remaining) step of the quest. I contacted a GM and was told it had been removed! On a whim, I revisited each shard holder, and, sure enough, discovered I'd never actually finished the Arcanite Buoy nonsense because of a couple of bloody Blue Sapphire. I grabbed the gems, ran off, spent 15+ minutes fighting Neptulon, got my shard, got my staff, and got my achievement.
My favorite is easily Hand of Adal because you had to soldier through content that was actually difficult and when the game didnt just give you loot for fun.

The Undying. For sure. For one, its a pain to get so its a testament to both luck and skill. Two its unobtainable anymore, making it a rarer title. Lastly, the achievement rewards an awesome title.
For me it would have to be either Sarth25 3D, or Kingslayer. After such a long ordeal with Arthas, killing him kind of felt like the thing I'd been waiting for since I started playing.
